INFORMATION COLLECTION & USE
Collection of your personal information
To apply for our services, we require certain information, including but not limited to your first name, last name, date of birth, email address, allergies, and phone number. We will also require information related to your treatment, such as medications needed for administration, type of procedure planned, and physician details.
Other personal information, questions, documents, etc may also be collected via email, phone, or text message.
As is true of most websites, we gather certain information automatically and store it in log files. This information may include Internet protocol (IP) addresses, browser type, internet service provider (ISP), referring/exit pages, operating system, date/time stamp, and/or clickstream data. While not all of the information is personally identifiable, it may be associated with personal information that authorized users provide to us through our website. This information is held by our website host and will not be available or shared with Fertility Home Care.
Only authorized users can access and use our application form data. The information is transmitted to our encrypted, secure server for storage.
Fertility Home Care may also periodically send you promotional emails about new products and services. Any marketing communications sent will have an automatic opt-out link at the bottom of each communication. You may elect to be removed from any future communications by selecting and confirming this link or by contacting Fertility Home Care at email@example.com
PROTECTION OF INFORMATION
Fertility Home Care will not sell, trade, rent, or voluntarily provide to others any personally identifiable information collected during the application process or services. We will share the information provided to only the staff members involved with your treatment. We may disclose or share your personal or medical information, if reasonably needed, to other medical professionals, such as EMTs or doctors. We may also disclose such personally identifiable information that is required to be disclosed by law or by an appropriate law enforcement or governmental authority.
Information security and HIPAA security and privacy
A significant amount of the patient data maintained within our databases is personal health information (PHI) that is subject to the Health Information Portability and Accountability Act of 1996, as amended, and its implementing regulations, including the Privacy and Security regulations (collectively, the “HIPAA Regulations”). Fertility Home Care maintains consent agreements with our customers before services are rendered, which specifies our responsibilities and obligations to protect PHI and comply with HIPAA Regulations.
Fertility Home Care has implemented Information Security and Privacy Policies from unauthorized access, improper use, alteration, unlawful or accidental destruction and loss. All our employees receive security and privacy training and are bound by confidentiality obligations regarding the use of personal or medical information and may be subject to discipline including termination and criminal prosecution for failure to meet these obligations.
Customer account information and PHI may be copied, stored, and retained on our database servers or as part of our normal backup processes. Information is retained and never deleted. Database servers and backup information is subject to physical, electronic, and managerial security measures and access is restricted to authorized personnel only.

Update or removal of personal information
Although the personal information we collect for user accounts through our application form is limited and voluntary, you are responsible for providing Fertility Home Care with accurate and complete information, and for contacting Fertility Home Care if correction of the information is required. We will reach out to verify your information is correct prior to any services rendered.
